What We Do
Duplin County's CMARC program offers care management services to the children and their families to help improve social determinants of health through care coordination, support and provision of resources as needed.
Who We Serve
Children from birth to five years old with Medicaid who are considered high risk are eligible for CMARC. Those who are considered high risk are children with long-term medical conditions, infants who are in the NICU, children who live in stressful situations, those in foster care, and families who need linkage to community resources.
How We Impact
CMARC positively impacts our community by providing many families with an advocate for their child's health and well-being. Many families do not have adequate resources or are unaware of availability of those resources to help with food, transportation, housing, etc. Also, care management provides vital education to parents regarding safe sleep and car seat safety among various other topics.
