The Integrated Care Outreach Network (ICON) holds significant importance due to its relevance highlighted in the 2021 Community Health Assessment. Mental Health and Substance Use Disorder are identified as crucial areas impacting the quality of life for residents of Granville and Vance counties, as evidenced by the following statistics:
- More than 1 in 4 Granville County residents and 1 in 10 Vance County residents have been diagnosed with depression and anxiety.
- Suicide rates have risen by more than 50% since 2004 in both counties.
- 27% of Granville County residents and 38% of Vance County residents have a friend or family member affected by the use of prescription painkillers, opioids, or heroin.
In response to these challenges, ICON provides a solution by offering an integrated care and collaborative approach. This involves working in tandem with GVPH primary care and fostering the development of a network that involves community partners in Granville, Vance, and Warren counties. This coordinated effort seeks to address the mental health and substance use issues and promote overall well-being in the community.
