The goal is to help clients remain safe and healthy at home, without the need for emergency medical care.

Who We Serve
Any resident in Burke County, North Carolina.
How We Impact
- Care plan follow-up - helping patients understand doctors' orders, prescriptions and their correct usage, and proper use of medical equipment.
- Limited point-of-care testing such as ECGs, blood glucose checks, blood draws, etc.
- Referrals and assistance with in-home health, hospice and palliative care, substance use detox and rehab, mental health management and other outreach programs.
- Home hazards - identifying and reducing health and safety risks and improving home safety by arranging installation of grab bars, smoke alarms, etc.
Progress on CHIP
September 2023 to December 2023 progress:

Since October of 2023, there have been 39 referrals made

2 ED Diversions made from calls

6 individuals given Mental Health Resources who were seeking or were in need

In addition, there have been 5 individuals given Narcan
January 2024 to June 2024 progress:

approx 107 different individual referrals on the medical side (some multiple referrals),
-
approx 319 calls flagged as overdose/SUD (although a lot of them were not true overdoses or unfounded)
-
approx 48 Recovery Resource packets containing mental health resources
-
5 or 6 packets of strictly mental health information
-
12 ED diversions.
June 2024 to January 2025 progress:

76 different individual referrals on the medical side (some multiple referrals),
-
200 calls flagged as overdose/SUD (although a lot of them were not true overdoses or unfounded)

28 Recovery Resource packets containing mental health resources

17 ED diversions.
