Description
The Community Health Worker (CHW) Consortium is an initiative supporting Live Well Wake's work on increasing access to healthcare within Wake County. The Consortium was launched by Live Well Wake's Integrated Primary Care Subgroup, which focused on integrating CHW workforce into primary care models across the county. The consortium kicked off with the first event of the year on Thursday, August 22nd, 2024.
The mission of the consortium is to convene Community Health Workers across organizations within Wake County and surrounding areas in a supportive and collaborative forum to bridge access to health-based education, resources, and support equitable access to care. Moving forward, the consortium plans to convene stakeholders at in-person events on a quarterly basis.
Progress in 2024
The consortium kicked off with the first event of the year on Thursday, August 22nd, 2024, at the WakeMed Andrews Conference Center in Raleigh, North Carolina. The topic of the event was on affordable housing and homelessness in Wake County, one of Live Well Wake's priorities areas for Wake County.
