NCCARE360- Rollout

What We Do

NCCARE360 is North Carolina's groundbreaking statewide network that bridges health care and human services organizations through an innovative shared technology platform. This coordinated, community-focused, and person-centered solution streamlines the delivery of care across the state. NCCARE360 enables providers to electronically connect individuals with identified needs to local community resources, ensuring seamless communication, accountability, and follow-up for every referral. By closing the loop on referrals and reporting outcomes, NCCARE360 adopts a “no wrong door” approach to care. Available in all 100 counties, this resource is free for community-based organizations, social service agencies, local health departments, and other local partners, with subsidies available for major health systems and payors.

In Cumberland County, we leverage NCCARE360 in partnership with the Cumberland County Department of Health and Human Services. Together, we work with over 100 in-network organizations to provide timely and effective referrals, supporting the needs of our community members.

Who We Serve

Every day, many North Carolinians face challenges like food insecurity, housing instability, limited transportation, and other essential needs. NCCARE360 addresses these gaps by uniting health and human service providers across sectors, creating a seamless system of support. This innovative platform empowers individuals and communities, offering greater opportunities for healthier, more fulfilling lives. Born from a strong public-private partnership between the NC Department of Health and Human Services and the Foundation for Health Leadership & Innovation, NCCARE360 is implemented with the expertise of United Way of NC/2-1-1, Expound Decision Systems, and Unite Us.

Recognizing the unique needs of our military community, NCServes has partnered with NCCARE360 to deliver coordinated care and vital resources to North Carolina’s military populations, including those in Cumberland County. This collaboration ensures no one is left behind in the mission to build healthier communities.

How We Impact

NCCARE360’s community engagement team, powered by Unite Us, collaborates with community-based organizations, health plans, health systems, and government agencies to build a seamless statewide coordinated network. This dedicated team provides comprehensive support at every stage of joining the network, ensuring each partner feels empowered and well-supported. Even after new regions launch, the team maintains active engagement, working closely with partners to analyze data, evaluate network performance, gather feedback, and refine processes. They also offer continuous technical assistance, fostering a thriving and interconnected community.

Progress in 2022-2023

From 2022 to mid-2024, NCCARE 360 processed a total of 907 referrals, with 660 accepted and 247 rejected. In 2022, there were 593 referrals, 429 accepted and 164 rejected, with top rejection reasons being client ineligibility (61) and service unavailability (49). In 2023, out of 216 referrals, 158 were accepted and 58 rejected, with client ineligibility (30) and "other" reasons (9) leading. In 2024, 98 referrals were recorded, with 73 accepted and 25 rejected. The primary reasons for rejection in 2024 included lack of capacity (7) and client ineligibility (5). This data highlights ongoing trends in referral outcomes and barriers to service delivery.
