Diabetes Self-Management Education and Support programs can be found in hospital outpatient centers, clinics, physicians' offices, pharmacies, wellness centers, and more. These programs meet weekly for x number of weeks. DSMES programs are taught by healthcare professionals and meet criteria set by the US Department of Health and Human Services. Program participants gain knowledge, skills, and support needed to manage diabetes daily self-care.
As a result of completing the program, participants can:
- Understand exactly what diabetes is and how it affects their body
- Discover how diabetes medications work
- Identify what types of food are best for them and how to plan meals that fit their lifestyle, culture, religious beliefs and budget
- Determine the best type of glucose monitoring device for their specific circumstances
- Find apps and other tools that offer reminders and help them track their progress
- Help them cope with stress and solve problems as they arise
- Interpret data from their diabetes devices to get treatment recommendations
Progress in 2023-2024
The DSME program experienced staff turnover through 2023, influencing the frequency and type of class offerings. The second half of 2023 education sessions were held. Despite these challenges, the majority of participants were still able to lose/maintain weight and decrease their A1C.
