I. DATA QUESTIONS
1. Provide the estimated number and percentage of children in the neighborhood directly served by the grant program (i.e., total penetration rate).
2. Indicate how the total penetration rate and information relating to the metrics established under the Promise Neighborhoods Performance Indicators (GPRAs) have been made publicly available.
a. Provide the website address to the site where the information was made publicly available or provide a justification on why this information has not been made publicly available electronically.
3. If applicable, please describe any changes made to your GPRA targets.
4. If applicable, describe any data source updates you made in 2025 to previously reported GPRA data. For example, a grantee may update their MOU with the school district and can submit data for items previously not reported or may replace data from a source like an alumni survey with data from the National Student Clearinghouse.
5. List your active target schools and if applicable, describe any changes to the target schools listed in your application (e.g., school closures, new target schools, school name changes). Include the year when these changes occurred, school grade spans, and type of school (e.g., public non-charter, public charter, magnet).
6. Describe how charter schools located in or near the Promise Neighborhood footprint play a role, if at all, in your Promise Neighborhood. For example, do charter schools in the area impact student mobility, or does your Promise Neighborhood provide services to families whose children are enrolled in charter schools in the footprint?

II. GOVERNANCE STRUCTURE
1. Describe your organization’s governance, management structures, and Advisory Boards. Explain any changes to your organization’s infrastructure (primarily staffing) and systems. Include names and roles of new key staff (e.g., project director, data manager, cradle-to-career leads). If changes did not occur during the reporting period, please note this in your response.
2. List any changes in leadership since the start of your grant. Leadership roles may include changes in project directors, district superintendents, backbone agency CEO, and/or government representatives (e.g., mayor, governor, Tribal Council President/Chairman). Briefly describe how these changes affected your Promise Neighborhood and activities, including the years the changes occurred. This will help us understand the level of stability for key leaders associated with neighborhoods.

III. MISCELLANEOUS
1. What is your organization’s plan (e.g., financial sustainability, restructuring, leveraging community assets) for long-term population-level improvements after the grant ends? Please provide details on your sustainability plans, including a multi-year financial and operating model. For grantees in the 4th year (or beyond) of their grant, please estimate resources that have been secured and those still needed. Provide the commitment of any partners and evidence of broad support from stakeholders (e.g., State educational agencies, teachers’ unions) critical to the project’s long-term success. Be sure to discuss any modifications that may have occurred from your original application through the reporting period.
2. Provide a status update for each competitive preference priority in your application. Your status update must include successes and challenges associated with implementing these priorities during this reporting period.
