Description
A Behavioral Health and Intellectual/Developmental Disabilities (I/DD) Tailored Plan (Tailored Plan) is a North Carolina Medicaid Managed Care health plan. It offers physical health, pharmacy (prescriptions), care management and behavioral health services. It is for members with mental health needs, substance use disorders, intellectual/developmental disabilities (I/DD) or traumatic brain injuries (TBI). Tailored Plans offer added services for members who qualify.
Tailored Plans started July 1, 2024.
Progress in 2024
Since the rollout of the Tailored Plans in July, our Care Manager has seen multiple patients now eligible for the plan. These patients are now being followed by the LME/MCO care manger. The Yadkin County Medical Clinic has also entered into a contract with Carolina Complete Health to provide in-network primary care services for those on Tailored Plans in our region. The combination of Medicaid expansion and tailored plans have resulted in an increase of patients with chronic diseases now being served at the Medical Clinic.
Progress in 2025
In July 1, 2025 the new rate of reimbursement for care management related to tailored plans impacted many. These plans continue to cover physical, mental & behavioral health, pharmacy & specialized care--including TBI and the Innovations waiver. LME/MCOs include: Vaya, Partners, Alliance Health & Trillium.
