Catawba County Public Health utilizes NCCARE360, a statewide coordinated care network, to screen individuals for non-medical drivers of health needs, such as food insecurity, housing stability, and access to transportation, and to make referrals to other internal program areas and community partners to address identified needs. One of the largest identified needs is around food insecurity, aligning with our CHIP priority to incease access to healthy foods.
