Action Plan

FY21:

The CCFN Coordinator worked closely with Southern Maryland Community Network (SMCN) to build program capacity and to promote the program (TF-CBT and Youth Engagement) in the community. CCFN in partnership with SMCN developed an outreach and promotion plan for the program and monitored outcomes weekly. Below is a list of steps taken to establish the program as a valuable resource in Calvert County:             

  1. Calvert County Government's Department of Community and Media Relations developed professional flyers for each program component, in-kind. 

  2. CCFN Coordinator worked with TF-CBT Therapist streamline the referral process for families making a self-referral

  3. Developed weekly outreach plan—to include meetings with stakeholders, paid advertisements, social media post and speaking engagements.

  4. In FY21, the TF-CBT target population expanded to include youth impacted by trauma.

  5. The TF-CBT Therapist participated in Local Care Team meetings and provided education to families about the model and provided referral packets.

  6. Calvert County Government spotlighted the TF-CBT program in a weekly e-newsletter for Citizens.

The TF-CBT portion of the program will not continue in the second have of FY21.

 

The CCFN provided ongoing Technical Assistance for the Youth Engagement portion of the program in FY21. Due to low participation, this program will not continue in FY22.  

 

 

Program Summary

FY21: This two pronged program aimed at reducing the impact of Adverse Childhood Experiences, trauma, and incarceration on children, families and the community provides a continuum of services. A Master's Level trauma based Therapist provides therapeutic interventions for youth of incarcerated parents and/or youth impacted by trauma and their families. Youth Engagement services provide soft skills training and job search or educational/vocational program enrollment assistance.

Theraputic Interventions: Youth impacted by incarceration and/or with a history of trauma, to include family dysfunciton. 

Youth Engagement: Disconnected Youth ineligible for WIOA Services, and youth at risk of disconnection enrolled in the McKinney Vento program and/or foster care. 

FY20:                                                                                                                                                                            

Theraputic Interventions: Youth with a parent incarcerated at the Calvert County Detention Center and/or previously incarcerated.                                               

Youth Engagement: Disconnected Youth ineligible for WIOA Services, and youth at risk of disconnection enrolled in the McKinney Vento program and/or foster care.

HFY2 FY21: This program will not continue in FY22. 

HFY1 FY21:

The CCFN Coordinator worked closely with Southern Maryland Community Network (SMCN) to build program capacity and to promote the program in the community. CCFN in partnership with SMCN develop an outreach and promotion plan for the program and monitored outcomes weekly.

Even still, the Trauma Focused Cognitive Behavioral Thereapy (TF-CBT) component of the program did not serve any youth in the first half of FY21. On January 25, 2021,(SMCN) submitted a 30 day notice to terminate the TF-CBT portion of the program. SMCN cited the increased need for crisis related services for youth and families amid the COVID19 Pandemic. SMCN reports families are not seeking trauma focused treatment at this time. The CCFN Board will determine how to reallocate funds from this program for the second half of FY21 during thier February 22, 2021 meeting. 

The CCFN Coordinator continues to work closed with SMCN to build capacity for the Youth Engagement portion of the program.
