What We Do
Stakeholders agree that Rochester / Monroe County is “program rich and coordination poor” meaning that programs need to be better aligned to ensure person-centered and outcomes-oriented service. Shifts in the healthcare landscape to align with social and human services have created additional momentum for greater coordination among service providers. Connected and coordinated service provision ensures the system is person-centered at various levels: affordable, culturally appropriate resources are available to residents – and that residents are aware of these resources and can access them. Today, stigma exists around accessing resources – a future, person-centered system would ensure these resources increased individuals’ optimism about their future paths and opportunities.

How We Impact
RMAPI represents a neutral convener that can work in partnership with others to build on the momentum. RMAPI will work in close partnership with the Finger Lakes Performing Provider System (FLPPS) and the United Way to foster deeper relationships among service providers to coordinate a system of referrals and service delivery that results in “no wrong door” to person-centered services. This will include mapping the current system to understand points of disconnect and opportunities for partnership, as well as determining how partners can develop a single data set to track individual person-centered outcomes. This strategy will be implemented by the “Service Provision Working Group”.

Initial activities: |
- Understand the current system: map social supports availability / accessibility barriers in areas of concentrated poverty - Map actors and referral disconnects, especially between healthcare / social services - Identify navigation and policy solutions needed - Establish a process for data sharing: share current data collection approaches among agencies / organizations to map potential shared data set at individual and program outcome levels - Infuse guiding principles across partners: - Catalog current tools and trainings for trauma and structural racism - Understand which agencies / orgs have used tools / trainings and what gaps exist Coordinate work with FLPPS CBO Advisory Council to ensure work is distinct but complementary |
