Duplin County CMHRP provides care management services to pregnant and postpartum women who are considered to be high risk. These services include antenatal and postpartum education, assistance addressing barriers to healthcare, linkage to community resources and care coordination. 

Pregnant or postpartum women with and without Medicaid. Our county is very fortunate to provide care management services to those without Medicaid through vital funding and it is very much needed. Our county has seen a rise in foreign-born patients over the recent years whom do not qualify for Medicaid but are in dire need of resources, education and care coordination.

Care management services for women with high risk pregnancies produces positive patient outcomes and provides support to patients who may be in need of extensive care coordination and education regarding pregnancy, childbirth and postpartum care. CMHRP provides mothers with resources needed for transportation to their appointments, housing needs, food insecurities and much more.
