Description
Care Management for At-Risk Children is a program offered at no charge for eligible children from birth to 5 years of age who:
- Have chronic medical conditions
- Are in stressful situations long term
- Have been referred by the child’s doctor
The goal of CMARC is to improve the health of your child by linking you to effective, efficient services that will meet the needs of your child and family. For eligible families who want CMARC services, a care plan is designed by the family and the care manager.
Yadkin County has 1 full-time Care Manager who operates both CMARC and CMHRP programs; the program also has a part-time Community Health Worker to assist with follow-up and referrals for clients.
Progress in 2024
The CMARC program increased to an active caseload of 43 total clients in 2024. The addition of a part-time Community Health Worker (CHW) has proven beneficial as the caseload continues to trend upwards.
