Care Management for At Risk Children (CMARC)

Description

Guilford County Division of Health’s Care Management for At Risk Children (CMARC) program provides free case management for children up to five years of age who are born with chronic conditions, at risk for developmental delays or have been impacted by social determinants of health/toxic stress or trauma. Specially trained nurses and social workers coordinate visits with the parent or guardian to develop a comprehensive care plan with goals to address any identified concerns or risk factors.

Services include:

  • Helping the caregiver communicate their needs and concerns with doctors, specialists, and other professionals working with the family
  • Educating the family on Safe Sleep Practices, the importance of Well Child Checks, and Early Dental Care
  • Scheduling medical appointments
  • Exploring childcare options
  • Making referrals to other agencies and programs
  • Providing information regarding age-appropriate growth and development. Nurses and social workers complete periodic developmental screenings until the child turns five years of age

Progress in 2024

From January through December 2024, CMARC staff enrolled 1,078 new members and completed 1,342 face to face visits. At these visits, care managers completed comprehensive needs assessments and developed tailored care plans with goals for families. The Care Managers also delivered essential supplies such as infant formula, diapers, safe sleep equipment, children’s clothes, and food to the families. In 2024, CMARC also received 118 Plan of Safe Care-Opioid /Substance Exposed Infant referrals from Child Protective Services that were assigned for outreach.
