The Family Connects Guilford program offers postpartum/newborn nurse home visits to all Guilford County residents following the birth of a baby. Following the Family Connects International evidence-based model for implementation, nurses with advanced knowledge and experience in public health, perinatal nursing, and lactation provide support, education, and early identification and referral of health and safety concerns. Visits include a thorough physical assessment of mom and baby, psychosocial assessment of family and environment, referral, and follow up of identified needs and educational counseling. Interpreter assistance is provided to Spanish-speaking families and for other languages utilizing a Language Line provider.

For the 2024 calendar year, Family Connects Guilford (FCG) staff engaged 2,679 families with new infants and completed 2,133 home visits providing support during the early postpartum and newborn period. FCG nursing staff serve all families in Guilford County including the 27260 and 27262 zip codes, where low birth weight and infant deaths are more concentrated.

Of the home visits provided, 2,050 families had an identified risk, and 2,893 referrals were made to assist these families. Identified risks include needed material supports, health care needs, assistance in caring for an infant, providing a safe home, and additional supports for parents.
