Community Screening/Education is a community-based screening effort to identify hypertension in participants. The program offers access to screenings in the community for continued blood pressure screenings, and continued education around the prevention and monitoring of Hypertension.
Atrium Anson Carolinas Primary Care (CPC) partners with local faith communities, community health, and local businesses to provide events for community blood pressure screenings and education: nutrition, exercise, and lifestyle changes.
Atrium Anson Carolinas Primary Care (CPC) provides educational resources needed to provide community participants with self-empowering tools for prevention and reduction of high blood pressure.
The program is evaluated by the number of community participants who are screened for high blood pressure and the number of educational sessions provided to the community.
Community Screening/Education is eligible for all Anson County community members.
Progress in 2023
In addition to monitoring quality metrics at Carolinas Primary Care, Atrium Health Anson’s staff is involved in community health events in the county throughout the year to provide screenings. In 2023, they screened over 1,500 residents at various public events other than the primary care setting. These events took place all over Anson County including Peachland, Morven, Burnsville, Wadesboro, Polkton, and Lilesville. Atrium also has a Mobile Primary Care Unit that is in Anson County four times a month that sees patients on a walk-in basis.
