What We Do
This program is a free Diabetes Awareness education series that is offered by the Nash County Health Department and Harrison Family YMCA two times per year. The curriculum is taught by Diabetes Lifestyle Coaches who teach participants the following:
-Learn the difference between Prediabetes, Types 1, 2, and Gestational diabetes
-Understanding Blood Glucose Levels and A1C Levels
-Learn about healthy eating, portion control, and healthy recipes to control diabetes
-Learn about counting carbohydrates and fats
-Learn about physical activity benefits
-Learn about managing Stress
-Learn about the importance of diabetes medication management
Who We Serve
The program is open to the community. All individuals age 18 and older are encouraged to participate, but those who have prediabetes, type 1 and 2 diabetes and highly encouraged to participate in the program sessions. The program is limited to only 25 per class. All sessions are held on the 2nd Monday of each month from 5:30 pm. to 7:00 pm. Individuals must register for the class through the Harrison Family YMCA or Nash County Health Department. Currently, all seasons are held virtually due to the COVID-19 paramedic and staff will inform about face-to-face options. Individuals will receive information brochures and program incentives for their participation.
How We Impact
-Improve individuals access to diabetes awareness for diabetes self-management
-Reduce the number of hospitalizations from uncontrol diabetes
-Improve individual's knowledge about healthy eating and physical activity to control diabetes
-Reduce the number of premature deaths from uncontrol diabetes
Progress in 2023
- Program was paused in 2022 due to staff changes. Therefore, no progress was made with this program in 2023.
- However, Nash County Health Department hopes to continue this program and partnership in the future.
