Data Description: Peoples’ physical and mental health directly impacts their ability to participate in daily activities including work and play. Increased days of poor physical and mental health can indicate the need for services and programs that address these measures. The NCCHW supports programming that addresses physical health (ex. chronic disease management, falls prevention) and mental health (ex. Social Isolation) to decrease the number of days that people are unable to participate in their daily lives.
