Agencies that closely monitor people's safety provide high quality better and promote better outcomes for the people that they serve and support.
0 - Agency does not meet criteria as described by designation rules regarding client safety
1 - Agency has (1) policy in place that stipulates a systematic process for identifying and reviewing client safety-related incidents, AND (2) can provide documentation of staff training re same. *Examples of client safety-related incidents include serious injuries, medication errors, instances of restraint or seclusion, duty to warn, risk of dangerousness to self or others, and other critical incidents. Agency can provide evidence that its policy is enacted as written. *Examples of evidence might include a database/spreadsheet used for trend analysis, meeting minutes, and training records. For example: agency keeps track of restraints and seclusions within a program and reviews this regularly at the programmatic level.
2 - Agency can demonstrate either (1) a higher-level systems analysis of client safety and/or (2) evidence of systematic provision of safety education to staff and persons served based on systematic identification and review of data. For example: Restraints and seclusions are reviewed at a programmatically, across an agency, and are reviewed by an entity outside of the originating program. A score of "2" indicates that there was action taken as a result of the analysis of the safety data. Examples of "action taken" could include programmatic changes, policy and/or procedural changes.
