Story Behind the Curve
Children who have experienced a psychiatric hospitalization are at risk for readmission if supports are not provided in a timely manner upon discharge. Centers of excellence are aware when the children and youth they serve have been placed into a psychiatric hospital so that they are able to coordinate their care prior to discharge and then support the children and youth after discharge. For this measure, this activity includes a psychiatric hospital and any use of PNMI funds that result in a placement outside of the home due to a mental health condition.
0 – Criteria not met (Agency does not provide any evidence that they provide coordinated care during a psychiatric stay or upon discharge)
1 – Criteria partially met (Agency provides evidence that they are aware of discharges but these processes are not memorialized in agency policies and procedures) Policies and procedures available
