Story Behind the Curve

It is important to provide regular follow-up supports to people who have experienced the need for higher level care to prevent readmission into the higher level of care. Support following an acute placement promotes a smooth transition into the community and reduced likelihood of readmission.  This measures considers the percentage of people whose primary program of service is Developmental Service, whether within a designated agency or an SSA, who are seen within 7 days after a discharge from a psychiatric or other crisis residential admission (I/DDVCIN, psychiatric hospitalization, and/or agency crisis placement) by a provider of the agency.   Follow up includes any type of agency support or services that is billable or that would be recorded as an encounter.

1 – Agency provides follow up care for 100% of people within 7 days as indicated by agency data

2 – Agency provides follow up care for 100% of people within 7 days as indicated by agency data pulled from the electronic health record
