What's Hurting?

  • Thirty-six percent of Sampson County adults are obese and 29% of adults are physically inactive (2023 County Health Rankings).
  • During 2020-2021, the program was put on hold due to the COVID-19 pandemic and public health's focus on COVID-19 response and preventative efforts.
  • Cultures can differ in many ways and can influence healthy eating and physical activity.

What Works

Clients receive nutritional education to include:  food labels, portion sizes, counting carbs, My Plate, etc.

Clients receive physical activity education to include: benefits of physical activity, increasing physical activity, types of physical activity, etc.

Clients set SMART (Specific, Measurable, Achievable, Realistic, Timely) goals for effective diabetes management.

Action Plan

Continue providing DSMES program to clients with education on nutrition, physical activity, and monitoring blood sugar.

Assist clients with setting SMART goals.
