What's Helping?
- Focusing on self-care bahaviors in which participants learn ways to manage and improve their Hemoglobin A1C through better nutrition, physical activity, monitoring blood sugar, and medications.
What's Hurting?
- Cultures can differ in many ways and can influence the self-care behaviors associated with managing and improving one's Hemoglobin A1C.
- During 2020-2021, the DSMES program was put on hold due to the COVID-19 pandemic and public health's focus on COVID-19 response and preventative efforts.
- Gestational diabetes participants often have a lower A1C, both at the time of diagnosis and after completing education, than participants diagnosed with type II diabetes, thereby data may be lower.

What Works
Clients receive nutritional education to include: food labels, portion sizes, counting carbs, My Plate, etc.
Clients receive physical activity education to include: benefits of physical activity, increasing physical activity, types of physical activity, etc.
Clients set SMART (Specific, Measurable, Achievable, Realistic, Timely) goals for effective diabetes management.
Action Plan
Continue providing DSMES program to clients with education on nutrition, physical activity, and monitoring blood sugar.
Assist clients with setting SMART goals.
