Why Is This Important?

NCCHW is committed to employing various strategies aimed at achieving sustainability for these evidence-based programs. As part of that commitment, the Center will apply for six federal, foundation and/or community-based grants during the three-year project period with the aim of braiding and leveraging funding from other sources to achieve sustainability. 

  • During the first year of this project’s funding period, NCCHW sought and was awarded $409,196 in funding by the Administration for Community Living for an initiative to analyze the No Wrong Door (NWD) and Long Term Services and Supports (LTSS) system in North Carolina and to develop an implementation plan for an efficient No Wrong Door/Long Term Services and Supports system in North Carolina. The implementation of a more efficient and cohesive NWD/LTSS system in the state will provide much needed infrastructure and capacity for the delivery of EBPs statewide and for increased consumer awareness of program offerings. 

  • The Center also received funding in the amount of $453,282 from the North Carolina Division for Aging and Adult Services (Department of Health and Human Services) to sustain and expand its program to address social isolation and loneliness in older adults and to provide technology support to address the digital divide.

  • As part of a falls prevention grant, NCCHW also received funding to facilitate access to Arthritis Appropriate Evidence Based Interventions (AAEBIs) through our Healthy Aging NC (HANC) community care hub (CCH).

  • The Center has also submitted a proposal to the Centers for Disease Control and Prevention that would provide three years of funding for chronic disease self-management for North Carolinians with arthritis.
