Story Behind the Curve
Gaston County Public Health aims to involve community partners in working towards the Access to Health and Neighborhood Health priority in the CHIP. This effort seeks to bring diverse perspectives and experiences, leverage local expertise, and drive resource mobilization for the implementation and sustainability of health improvement initiatives within the community.

What Works
Gaston County Public Health actively collaborates with diverse partners by facilitating coordination through multiple planning meetings. Together, we engage in the collaborative planning of community events and contribute ideas to formulate actionable plans for implementation.
Action Plan
We plan to continuously engage with the Mountainview neighborhood by hosting meet-and-greet events, health resource sessions, and community meetings at their space. Our goal is to build community relationships, connect residents with county resources, and provide health education. We are focusing on this neighborhood because it falls within one of our high-health and social risk census tracts, but we aim to expand these efforts to other communities in the future. Connecting with other neighborhood-focused partners will be part of our future plans. Targeting one community at a time will give us the opportunity to pilot this newly developed initiative.
