Data about direct family contacts are obtained from all Connecticut Medical Home Initiative partners, see list below. Direct Family Contacts are counted by interactions using phone, email, social media or in person/virtual meetings.
CT Medical Home Initiative contractors provide care coordination for Children and Youth with Special Health Care Needs and their families/caregivers determined to be eligible under the State Title V Children and Youth with Special Health Care Needs (CYSHCN) Program; and work in partnership with pediatric primary care practitioners and CYSHCN and their families/caregivers in accessing needed services.
These data are current as of year end 2021. It is anticipated that data will be updated annually with the next update being by the end of 2nd Quarter of 2023.

Action Plan
All contractors including hospitals and community organizations, provide services through the Connecticut Medical Home Initiative (CMHI). They will continue to link CYSHCN and their families to any services or providers that they need. The involvement and satisfaction of families in the care of their CSHCN is essential to ensuring consistent and quality care. Ensuring that families are partners in decision-making at all levels including the healthcare team and supporting parent leaders to be actively involved in policy development results in improved outcomes for children and the system of care.
