Story Behind the Curve
The Level I plan of care (POC) is a client-specific, comprehensive care/service plan document that must be developed by a registered nurse or licensed social worker in partnership with the family/client. The Level I plan of care is a client-specific, comprehensive care/service plan developed once within an eligibility year. It is comprised of client demographics, insurance coverage, medical, social, educational, and functional strengths or needs, medical providers, medication taken and medical supplies needed. Problems and goals are developed based on the information obtained.
CSHCS data is collected on a fiscal year calendar (October 1 - September 30). Note that 2023 data is provisional (11/13/2023)

2023 Plan of Care QI Project
Currently the CSHCS Plan of Care (POC) is printed, mailed to the family for signature and returned to the office. The process is cumbersome, the document is long with limited formatting options and the return rate of signed POCs is low. The project will be implemented and piloted in FY 23 with the goals to to improve the formatting and completion of a POC by CSHCS nurses, and to increase the rate of signed, returned POCs with families.
PHASE 1: Conversion of paper Plan of Care to digital POC - in progress
