Story Behind the Curve

People who have a primary care provider are less likely to use the Emergency Department for routine care and receive higher quality care overall.  Many people supported by network agencies live with complex medical needs that require regular monitoring to maintain their health and prevent other health complications.    In this measure, agencies are able to provide evidence that they ask people in their preferred mode of communication whether they have a primary care provider (PCP).

0 - up to 79% and/or no policy is provided requiring staff to ask whether a person has a PCP

1 - 80 – 89% of new intakes asked whether they have a PCP as evidenced by a query from the Electronic Health Record.  A policy requiring staff to ask whether a person has a PCP is required

2 - 90 – 100% of new intakes asked whether they have a PCP as evidenced by a query from the Electronic Health Record.  A policy requiring staff to ask whether a person has a PCP is required.
