While some patients have on-going care plans that include multiple hospitalizations, many hospital readmissions are avoidable. Avoidable readmissions reflect health risks to patients and unnecessary health care costs.
This indicator measures the share of Medicare patients who are discharged from a hospital and readmitted within 30 days to the same hospital or another acute care hospital. High rates indicate a need to improve care coordination and discharge plans, including patient and caregiver engagement.
