Notes on Methodology

  • The target trendline in the graph above represents an accepted national benchmark percentile for Medicaid programs across the country. The value graphed historically was the national 50th percentile for comparison purposes. In 2020 (for measurement year 2019) we modified the value shown to demonstrate a target based on our actual performance.  For this measure, the target is the 25th national percentile.
  • Please note that a lower rate is better for this measure.
  • The CDC measure is a hybrid HEDIS measure; it requires both administrative claims data and data abstracted during a medical record review. The DVHA completed a medical record review for this measure for the first time in 2014. The diabetes in poor control measure was first produced by VT Medicaid in 2019.

Story Behind the Curve

For this measure, lower rates are better.

This composite measure, with 7 different elements including this one for HbA1-c poor control, looks at how well Vermont Medicaid cares for the common and serious chronic disease of diabetes. Diabetes is one of the most costly and highly prevalent chronic diseases in the U.S. Many complications, such as amputation, blindness and kidney failure can be prevented if detected and addressed in the early stages.

Vermont Medicaid has developed a care management program, the Vermont Chronic Care Initiative, that helps Medicaid beneficiaries with the most need, including those with diabetes, to better manage their conditions. Please see Partners and Action Plan below for more information.
