The agency provides comprehensive and collaborative support to its contracted Shared Living Providers (SLPs).

0 – Criteria not met (Agency can document no or little evidence of active collaboration, or high quality training and support)

1 – Criteria partially met (Agency can document some evidence of active collaboration, and/or high quality training and support, but evidence does not suggest high prioritization and consistent implementation)

2 – Criteria met (Agency has clear policies and procedures and can provide evidence that the outcome is a consistent, high priority.   Examples of evidence may include policies and procedures, training requirements and opportunities (threshold of initial trainings followed by one annual training per SLP), SLP involvement in training, evidence of SLP involvement in relevant committees, and the presence of a peer support network of SLPs.)
