The San Francisco Health Plan (SFHP) is a managed care health plan whose mission is to improve health outcomes for its over 145,000 members.* Eight out of every ten people eligible for Medi-Cal in San Francisco choose SFHP to cover their preventive care, specialty care, hospitalization, prescription drugs, and family planning services. SFHP HCV treatment completions is important to track as this plan serves the majority of low-income San Franciscans, a population who bears an undue burden of new HCV diagnoses and experience more barriers to care.
