Story Behind the Curve
The indicator measures the number of Local Health Departments in Indiana who have an agency Community Health Improvement Plan.
The creation of a community health improvement plan is a collaborative process between the local health departments, partners, and other stakeholders which identifies priorities the group can collectively work to address.

What Works
Conducting or participating in a Community Health Assessment encourages partnerships and on-going collaboration with the community and stakeholders in our local communities. Agencies can align services, share resources, and use the data to make informed decisions on priorities of the partnership and the agencies individually.
Challenges
There are several challenges for local health departments to be able to facilitate or participate in this process. These included but are not limited to:
- Funding
- Knowledge of the process
- Utilization of other Community Health Assessments (CHAs) in the area, such as those by local hospitals, and a Community Health Improvement Plan (CHIP) is not created
Corrective Action
Building relationships and educating LHDs the importance of facilitating or participating in the development of a Community Health Improvement Plan.
