Notes on Methodology
Any transition/discharge that does not involve a hospital or correctional facility is considered a lower level of care. This includes residential care homes, nursing homes, individual or shared apartments, and living with family. If a resident discharged to a hospital or correctional facility, this would be considered discharging to a higher level of care.
