This is a new performance measure as of May 2023. As such, a limited number of partners are reporting this data point currently:
- ACPC: Agencywide
- MGMC Home Health: Community Clinics
- YSS: Service Coordination - This includes referrals both to external (non-YSS) and internal mental health programs.
It is anticipated additional partners will start to report their referrals in FY24.
