Who are the Partners that have a role to Turn the Curve? (Address the priority factors)

LOHP - Taking lead on the program and objectives.

EDCOE - School nurses/staff as partners in distributing information, creating forms, collecting data, helping with on-site screening process, follow up with referrals, sending final KOHA data to EDCOE staff that will enter numbers into SCOHR.

RDHAP - responsible for screenings and populating findings on KOHA forms.
What Works
What Works to Turn the Curve? (Actions to address the priority factors)

Since starting the program in February 2024, we have seen 12 out of 29 schools.

We screened 629 students out of 932 students enrolled
-
We identified 463 students without a need for dental care, 123 students with an early need for care, and 42 students with an urgent need of dental care.
-
6 students with an urgent need for dental care were referred by the school nurse to our MCAH program for care coordination, and 5 families have been helped to care and further treatment.
