Partners
Community-based organizations, as a requirement of funding acceptance, either collect and report on individual data or aggregate data based upon the type of intervention/program being implemented. Quarterly, as required by the Ohio Department of Medicaid, funded partners report the number of newly enrolled clients/participants by race.

PERFORMANCE MEASURES COLLECTED ARE DIRECTLY RELATED TO GRANT DELIVERABLES ESTABLISHED IN PARTNER GRANT AGREEMENTS. AGGREGATED DATA WILL BE REPORTED QUARTERLY, OR BIANNUALLY, OR ANNUALLY BASED ON ABILITY TO MAINTAIN CONFIDENTIALITY.
