This performance measure,  quantifies the proportion of all the offices cases that undergo a structured review process to identify opportunities for improving operational and investigative quality. 

The central purpose of this measure is to assess the commitment of the Office of the Medical Examiner (OME) to continuous quality improvement (QI). A systematic case review process is vital for ensuring the accuracy, consistency, and integrity of the death investigation and autopsy processes. By regularly reviewing a subset of cases, each unit (e.g., investigators, pathologists, and toxicologists) can:

  • Identify procedural deviations or areas where protocols are not being followed.
  • Pinpoint systemic issues that may lead to errors, delays, or inconsistencies in reporting.
  • Highlight opportunities for training and professional development.
  • Promote a culture of accountability and excellence across all departments.

A higher percentage indicates a more proactive and robust internal quality assurance program, which in turn strengthens the credibility and reliability of the OME's findings.
