Story Behind the Curve

Hint: To best organize content, create subsections using the following headers and organize related factors underneath each: Positive (or Contributing) Factors, Negative (or Restricting) Factors, Anticipated Factors, and Additional Research. List factors in order of priority based on impact and include both internal and external factors, when possible.

Contributing Factors: (i.e., Factors that move the curve in the desired direction)
  • Factor 1 Title: An explanation of how this factor impacts the slope of the trendline.
  • Factor 2 Title: An explanation of how this factor impacts the slope of the trendline.
  • Factor 3 Title: An explanation of how this factor impacts the slope of the trendline.
Restricting Factors: (i.e., Factors that prevent the curve from moving in the desired direction)
  • Factor 1 Title: An explanation of how this factor impacts the slope of the trendline.
  • Factor 2 Title: An explanation of how this factor impacts the slope of the trendline.
  • Factor 3 Title: An explanation of how this factor impacts the slope of the trendline.
Anticipated Factors: (i.e., Factors that influence the forecast part of the trendline)
  • Factor 1 Title: An explanation of how this factor impacts the forecast of the trendline.
  • Factor 2 Title: An explanation of how this factor impacts the forecast of the trendline.
  • Factor 3 Title: An explanation of how this factor impacts the forecast of the trendline.
Additional Research: (i.e., Additional research needed to surface root causes)
  • Agenda Item 1: Research item needed to surface additional factors that impact the curve.
  • Agenda Item 2: Research item needed to surface additional factors that impact the curve.
  • Agenda Item 3: Research item needed to surface additional factors that impact the curve.

Partners

Hint: List the partners and briefly describe the role that each will play to help turn the curve.

Additional Research: (i.e., Additional research needed to surface root causes)
  • Partner 1: An explanation of the role that this partner will play to help turn the curve.
  • Partner 2: An explanation of the role that this partner will play to help turn the curve.
  • Partner 3: RAn explanation of the role that this partner will play to help turn the curve.

What Works

Hint: To best organize content, create subsections using the following headers and organize related content underneath each: Research/Evidence-Based Practices, Low Cost/No Cost Solutions, Off-the-Wall Ideas (or Innovations), and Additional Research.

Research/Evidence-Based Practices: (i.e., Evidence-based practices that may move the curve in the desired direction)
  • Evidence-Based Practice Title 1: An explanation of the research/evidence-based practice.
  • Evidence-Based Practice Title 2: An explanation of the research/evidence-based practice.
  • Evidence-Based Practice Title 3: An explanation of the research/evidence-based practice.
Low Cost/No Cost Solutions: (i.e., Resourceful solutions that may move the curve in the desired direction)
  • Low Cost/No Cost Solution Title 1: An explanation of the low cost/no cost solution.
  • Low Cost/No Cost Solution Title 2: An explanation of the low cost/no cost solution.
  • Low Cost/No Cost Solution Title 3: An explanation of the low cost/no cost solution.
Off-the-Wall Ideas (or Innovations): (i.e., "Outside the box" solutions that broaden our thinking on what may be possible)
  • Off-the-Wall Idea Title 1: An explanation of the off-the-wall idea.
  • Off-the-Wall Idea Title 2: An explanation of the off-the-wall idea.
  • Off-the-Wall Idea Title 3: An explanation of the off-the-wall idea.
Additional Research: (i.e., Additional research needed to help surface new ideas that will help turn the curve)
  • Research Agenda Item 1: An explanation of the research needed to surface new ideas that can help turn the curve.
  • Research Agenda Item 2: An explanation of the research needed to surface new ideas that can help turn the curve.
  • Research Agenda Item 3: An explanation of the research needed to surface new ideas that can help turn the curve.

Strategies

Hint: Review formatting guidelines featured in the rubric. When outlining strategies each description should address the following questions: What will be done, Who will be involved in implementing the strategy (e.g., partners), When will it take place (e.g., what time period), How will it be done (e.g., describe how the strategy will be implemented), and How the strategy impacts the curve (e.g., leveraging the story behind the curve). Note: It is not necessary to include the Criteria for Selecting Actions Table in this section, (i.e., Leverage, Feasibility, Specificity, Values). If you’d like to include it, please add it to an appendix.

  • Strategy Title 1: An explanation of the strategy and how it is designed to turn the curve of the performance measure.
  • Strategy Title 2: An explanation of the strategy and how it is designed to turn the curve of the performance measure.
  • Strategy Title 3: An explanation of the strategy and how it is designed to turn the curve of the performance measure.
