The vast majority of referrals are triaged by the CIS Coordinator, with the exception of a small amount of referrals who may receive a home visit before the weekly referral and intake meeting.

CIS Team Referral sources - doctors, Nurse Family Partnership, hospital, mental health agencies, child care programs, State programs, schools, parents/families

• Weekly CIS team meeting is used to review all referrals.
• Outreach to community partners/referral sources.
• Team members who directly received referrals bring them to the team meeting to ensure that all referrals are received and processed.
• Have a clear written transition process for cases that started outside CIS, in particular nursing, and transition to CIS.

• Develop clear back up referral process plan when CIS Manager is unavailable. Action taken: Plan has been created and implemented.
• Continue outreach efforts.
• Continue weekly team meetings with members bringing referrals.
• Team members who directly received referrals bring them to the team meeting to ensure that all referrals are received and processed.
