This measure was revised in May 2023 to reflect the # of individuals referred to financial assistance and other non-food programs. - MGMC Home Health - Community Clinics (as of FY23)
- The Bridge Home/Emergency Residence Project (Shelter, Transitional Housing, Rapid Rehousing), MICA (SCFD Food Pantry)
- The Salvation Army (agency-wide, but mostly coming from Homelessness Prevention program)
- YSS (Transitional Living, Outpatient Service Coordination).
