Story Behind the Curve
Baltimore County HHS has set goals to increase the knowledge about HIV infections, and to increase the percentage of those with HIV viral suppression. By June 30, 2021, the HIV Case Management staff will identify clients who have a viral load > 10000 copies/mL and provide education regarding medication adherence to 75% of these clients. Reducing incidences of STDs such as HIV helps shrink costly direct medical care expenses and greatly reduces comorbidities.
Partners
Medical Providers:
Regular and ongoing case management appointments and continuous lab work monitoring work. Case managers provide health education including medication adherence during regularly scheduled appointments with clients enrolled into the case management program. Case managers obtain recent lab work from medical providers in order to monitor clients’ progress in achieving viral suppression.
What Works
Strategy
All clients in the HIV case management program will obtain Viral Load CD4 lab tests to monitor viral suppression. Clients that are identified as having a viral load > 10000 copies/mL will be contacted by a Program medical case manager and receive education on the importance of medication adherence.
