Death rates vary by county as seen in the map below and often correlate with historically underserved populations. Disparity data by race/ethnicity and gender is also available. While diabetes is a primary focus of this grant, the strategies also contribute to turning key curves across other chronic diseases; however a statewide comprehensive indicator for chronic disease mortality and morbidity is not currenlty available.
