Story Behind the Curve
The National Diabetes Prevention Program was established at the Bear River Health Department in 2016. This one year, evidence based program was created in 2010 to address the increasing burden of prediabetes and type 2 diabetes in the United States. The program focuses on lifestyle change, specifically healthy eating and physical activity. The goal is to lose 5-7% of your body weight and get 150 minutes of physical activity each week. By making these changes it is proven to reduce the risk of developing type 2 diabetes by 58% (71% for people over 60 years old).
Partners
The BRHD works with Intermountain Logan Regional Hospital, Medicare, CoMagine, Utah Department of Health and multiple health care providers to implement this program.
What Works
The National Diabetes Prevention Program is evidence based and proven to reduce the risk of developing type 2 diabetes by 58% (71% for people over 60 years old).
Strategy
Action Plan
Our goal is to conduct 8 cohorts annually, 4 cohorts in Box Elder County and 4 cohorts in Cache County annually. We aim to enroll participants who are on Medicare for two reasons the program is 100% covered by Medicare and as stated above those over 60 years old are more likely to reduce developing type 2 diabetes.
