What does it measure?
This measure provides the proportion of people who exit street outreach programs with a positive outcome. Positive outcomes include moving indoors to temporary housing, a higher level of care, or permanent housing, and excludes exits to places not meant for human habitation, jail, prison, or juvenile detention facilities.
How does it measure?
Question 23c on the Annual Performance Report, “Exit Destination" provides information about the number of individuals—adults and children— active in the date range who exit with positive outcomes, including:
- Permanent destinations
- Indoor temporary destinations such as emergency shelter, transitional housing, family and friends
- Institutional settings including foster care, psychiatric hospital, substance abuse treatment, hospitals, or long-term care facilities
The numerator in the equation is the sum of the number of individuals who moved into a one of the above situations recorded in the table. The denominator is the sum of the total number of persons from each table minus the total persons whose destinations exclude them from the calculation. Excluded from the calculation are persons who are deceased.
This performance measure is reported in cumulative, quarterly reporting periods that begin October 1st and end September 30th. The target is an annual goal.
