Implemented in 2018, the Health & Human Services Customer Service survey is collected at: https://exwchhs.qualtrics.com/jfe/form/SV_9tALTcaonyFbadw.  Employees include the survey link in their email address signatures, surveys are periodically handed out at the front desk when services are provided and the link is posted on our website.
