Challenges
There are several challenges for local health departments to be able to facilitate or participate in this process. These included but are not limited to:
- Funding
- Knowledge of the process
- Utilization of other Community Health Assessments (CHAs) in the area, such as those by local hospitals
Corrective Action
Health First Indiana will require all opt-in Local Health Departments to conduct or facilitate and Community Health Improvement Plan (CHIP.)
