Conducting or participating in a Community Health Assessment (CHA) encourages partnerships and on-going collaboration with the community and stakeholders in our local communities. Agencies can align services, share resources, and use the data to make informed decisions on priorities of the partnership and the agencies individually.

Challenges
There are several challenges for local health departments to be able to facilitate or participate in this process. These included but are not limited to:
- Funding
- Knowledge of the process
- Utilization of other Community Health Assessments (CHAs) in the area, such as those by local hospitals
Corrective Action
Health First Indiana will require all opt-in Local Health Departments to conduct or facilitate and Community Health Improvement Plan (CHIP.)
