Social Determinants of Health Literature Summaries
Healthy People 2030 define Social Determinants of Health (SDOH) as "the conditions in the environments where people are born, live, learn, work, play, worship, and age that affect a wide range of health, functioning, and quality-of-life outcomes and risks." Social determinants of health, such as safe housing, stable employment, transportation, and access to healthy foods, impact people's health, well-being, and quality of life.
Since August 2022, Onslow County Health Department screens all Health Department patients for food insecurity, safe housing, transportation needs, and physical/emotional safety using state mandated questions. If needs are identified through this screening process, clinical staff work with patients to make referrals to the Methodist Home for Children Prevention Specialists. The Prevention Specialists work with patients to identify needs and connect to resources to help meet patients' needs.
Monthly referrals will vary, as referrals are only made if patients indicate needs during the state mandated questions or during their visit.
