Measure Definition
This measure is still in development.
The total number of changes or improvements made within the year (minimum of four). These improvements could include; policy changes, process enhancements, resource allocation, training and development programs, communication strategies, and health and wellness strategies. Data collection can include; documentation, surveys and feedback, performance metrics, human resources records, leadership and stakeholder interviews. Reporting includes summarizing the number and types of interventions, along with any data on their effectiveness (if known).
