EA.5: People assigned to a mental health, ADAp, or emergency service program are seen for treatment within 14 calendar days for the completeion of the assessment. 

People who receive continuous care are more likely to remain engaged in care.  The percentage of people with an intake assessment who receieve follow up for an clinically indivated service other than an emergency crisis services within14 calendar days.

Note:  Success by Six programs are excluded from this calculations because they operate under a different referral process.

Story Behind the Curve

This outcome is measured within the Calendar Year.  The % of clients seen for treatment within 14 calendar days reflects our ability to get people into the right level of care as soon as possible after assessing their needs and strengths.  In Q1 and Q2 of 2020 we worked hard to look at workflows and capture this data more consistency.  Then in September of 2020 we switched to a new EMR which disrupted our workflows inpacting our numbers for Q4 2020.  We should start to see this number to start to increase again in Calendar Year 2021.

0 – Criteria not met (Agency does not collect the information electronically and/or agency provides a follow up service within 14 days of a completed psychosocial assessment less than 50% of the time)

1 – Criteria partially met (Agency provides a follow up service within 14 days of a completed psychosocial assessment 50 - 75% of the time as indicated by data extracted from the electronic health record)

2 – Criteria met (Agency provides a follow up service within 14 days of a completed psychosocial assessment 76% or more of the time as indicated by data extracted from the electronic health record)

 

The KPI dashboard screenshot below shows how we calculate and track our 14 day access numbers.  We use our KPI dashboards and filter it on the admission program and intitial contact date.  You will see this is the dame dashboard we use for our 5 day access and screening numbers it is all in one place and able to be downloaded and calculated.  We can then look over at the service code column and look for the 90791 assessment codes. The last column then calculates how many days have passed since that service date and the first service.  This way we can calculate the 14 day access numbers out of our system.  

 

The second evidence uploaded below is from our clear impact and show are historical numbers.  We did a lot of work here at NCSS right before Covid hit in order to increase our 14 day access numbers by making sure our clinicans were able to have room in their schedules to make sure they could follow up with in 14 days.  You will see once Covid hit and we adjusted to Covid our numbers dropped a little but stayed pretty consistant until our staff shortage started in Q4 of 2021 when we started seeing clinicians leave and having a hard time filling those open positions.
